Its a man in drag. In Japan people aren't crazy over homosexuality like the US so people can do stuff like that without being considered LGBT. Its probably a publicity thing- Are you more likely to remember that one bartender last Thursday or that zany bartender in drag?

People in Japan can dress up in drag without automatically being assumed they're Lesbian, Gay, Bisexual, or Transexual.
